Number one and the most important is the fact originally the game ended there and therefore what you saw was canon as in the developers plan for what happened next.

So without BS installed, everybody dies. However with BS and reason number two , obviously the game continues and would be pretty crappy if you were the only living one which in fact you would not be, since you are not immune to the virus as Eden thought.

The logical conclusion of this is that the virus takes time to work and after inserting the virus it takes time before all the mutates drop down dead you do see side effects, the Ghouls in underworld feel unwell, and you yourself will gradually get ill if you drink the water.

Its all down to how long passed originally between you inserting the virus and dying, and all the mutants following you. Since I can't recall of them stating a definate time frame other than the fact it happened , its doesn't take a huge jump of logic to assume that the original ending does still happen, but not right away.
